To report a claim you only need to send a written notice to the following contacts:

How to fill in the Report?
The claim report should include the following information:

• Policy number;
• Policyholder identity;
• Claim location;
• Contacts;
• Claim date and time;
• Detailed description of claim;
• Authorities intervention (say which);
• Identity of possible injured parties and relevant contacts;
• Identity of possible witnesses and relevant contacts.

In case of damaged machinery, the relevant brand and model should be detailed.
