How to fill in the Report?
The claim report should include the following information:

• Policy number;
• Policyholder identity;
• Claim date and time;
• Claim location;
• Detailed description of facts;
• Third party identity (name, address, taxpayer number, telephone, email);
• Technical fault report (copy);
• Detailed repair quotation (copy);
• Purchasing document related to the damaged good (copy);
• Photos of the claim location and damages;
• Witnesses (name, address, taxpayer number, telephone, email).
